Keeping Your Health in Mind
Community Healthcare you can depend on for adolescents (age 13 and older) to adults. Right now, all of our initial visits are scheduled remotely. Please make appointments online or make a phone call with our nurse practitioner. If you are having an emergency, please call 911.
